Coroner: "Glee" Actor Corey Monteith Died of Heroin, Alcohol Mix

Eddy Chen/FOX(VICTORIA, British Columbia) -- Several new details about the death of Cory Monteith have been released by the BC Coroners Service in Victoria, British Columbia, in its final report on the July 13 passing of the Glee star.

Coroner Claire Thompson concluded that Monteith died of mixed drug toxicity, involving intravenous heroin use combined with the ingestion of alcohol.

Monteith was found dead in his room at the Fairmont Pacific Rim hotel in Vancouver.  After the singer and actor failed to check out that night, hotel staff visited Monteith's room, finding the 31-year-old unresponsive.

According to the report, drug paraphernalia was found at the scene "including a spoon with drug residue and a used hypodermic needle."  In addition, two empty bottles of champagne were found.

The report notes that "video captured Mr. Monteith returning alone to his room at 2:16 a.m. on July 13, and that he was not seen alive again."

The actor completed a rehab program this past spring.  The final report continues, "It is noted that if someone has abstained from using opioids (which include heroin) for a period of time, tolerance may be lessened."

The death was ruled accidental.

Monteith, who portrayed Finn Hudson, will be remembered on a special episode of Glee, titled "Quarterback," on Oct. 10.
